To be eligible for Physicians Health Choice Wellness HMO Plan, you must:

• Be enrolled in Medicare Parts A and B
• Reside in our Covered Service areas:

   

  Austin Area
  Travis County
  Corpus Christi Area
  Nueces County
  Rio Grande Valley Area
  Cameron / Hidalgo

• Not be currently diagnosed with ESRD (kidney dialysis).
• Be diagnosed with Diabetes or Congestive Heart Failure

• No copayments for Diabetes Supplies and self-monitoring training.
• Orthopedic Shoes and Inserts
• No or low copayments for covered services and physician visits
• Inpatient Hospital Coverage
• Worldwide Emergency Coverage
• Medicare Part D Prescription Coverage

Physicians Health Choice is a Medicare Advantage Organization with Medicare contracts. Members must use contracting network pharmacies and providers. Limitations, co-payments and coinsurance will apply. Health plan premiums and benefits may vary by county and plan. Pharmacy benefits are limited to a formulary (a list of covered drugs).
