To be eligible for Physicians Health Choice Select HMO Plan, you must:

• Be enrolled in Medicare Part A and B
• Be enrolled in Medicaid
• Reside in one of our Covered Service areas:

  Travis / Williamson / Hays
  Corpus Christi Area
  Aransas / Nueces / San Patricio
  El Paso Area
  El Paso County
  Rio Grande Valley Area
  Cameron / Hidalgo / Willacy

• Not currently diagnosed with ESRD (kidney dialysis).
