To be eligible for Physicians Health Choice Basic HMO Plan, you must:

• Be enrolled in Medicare Part A and Part B.
• Continue to pay your monthly Part B premium.
• Reside in one of our Covered Service areas:

  Travis / Williamson / Hays
  Corpus Christi Area
  Aransas / Nueces / San Patricio / Jim Wells / Kleberg
  El Paso Area
  El Paso County
  Rio Grande Valley Area
  Cameron / Hildalgo / Willacy

• Not be currently diagnosed with ESRD (kidney dialysis).

Physicians Health Choice is a Medicare Advantage Organization with Medicare contracts. Members must use contracting network pharmacies and providers.
