Who is Physicians Health Choice?

Physicians Health Choice is a physician owned company. Our doctors were actively involved in creating this health plan to reflect a patient's needs. We are committed to bringing you more benefits than Original Medicare. When it comes to health care, you do have choices.

Physicians Health Choice is a Medicare Advantage Organization offering four health plans*:

Physicians Health Choice Standard HMO Plan is a Medicare Advantage plan for people with Medicare (Part A and Part B).

Physicians Health Choice Basic HMO Plan is a Medical Coverage Only Plan (with no prescription drug Plan).

Physicians Health Choice Extra HMO Plan is an enhanced medical and prescription drug Plan with a monthly premium.

Physicians Health Choice Select HMO Plan is for Medicare Beneficiaries with Medicare and Medicaid.

Physicians Health Choice Wellness HMO Plan is a Special Needs / Chronic Care Plan.

As a Medicare Advantage Organization, Physicians Health Choice has a contract with the Centers for Medicare and Medicaid Services (CMS), the government agency who runs the Medicare program. It is important to remember that Physicians Health Choice is not a Medicare supplement policy. (See Glossary of Terms section for definition of a Medicare supplement policy, commonly called a "Medigap"). As a member of Physicians Health Choice, you must use our network providers. This helps to ensure you pay only the required co-payment amounts for medical services you receive.
