To be eligible for Physicians Health Choice Extra Health Plan, you must:

> | Be enrolled in Medicare Part A and Part B.
|
|
> | Continue to pay your monthly Part B premium.
|
|
> | Reside in one of our Covered Service areas:
Texas
Armstrong County, Carson County, Gray County, Hutchinson County, Potter County and Randall County (Amarillo area)
Hays County, Travis County, Williamson County (Austin area)
Cameron County (Rio Grande Valley area)

> | Not be currently enrolled in an ESRD program (kidney dialysis).
|
|
|
> | Low monthly plan premium
|
|
> | No or low copayments for covered services and physician visits
|
|
> | Inpatient Hospital Coverage
|
|
> | Worldwide Emergency Coverage
|
|
> | Medicare Part D Prescription Coverage

Physicians Health Choice and Physicians Health Choice of New Mexico, Inc. are Medicare Advantage Organizations with Medicare contracts. Members must use contracting network pharmacies and providers. Limitations, co-payments and coinsurance will apply. Health plan premiums and benefits may vary by county and plan. Pharmacy benefits are limited to a formulary (a list of covered drugs).
