Welcome to the Physicians Health Choice Provider Section. This section provides information to prospective and existing providers. The Protecting Patient Privacy section is to enforce the importance of understanding the HIPAA requirements of using and disclosing patient information.

If you are a provider interested in becoming a part of our health plan, please select Join Our Network.

If you are an existing provider, select the Contracted Providers link to the left which will require a username and password. If you are a first time user, the system will prompt you to select NEW USER to set up your account. Upon completion of setting up your account, you will receive your password via email.
