To be eligible for Physicians Health Choice Standard HMO Plan, you must:

• Be enrolled in Medicare Part A and Part B.
• Continue to pay your monthly Part B premium.
• Reside in one of our Covered Service areas:

  Ft. Lauderdale and Palm Beach Area
  Broward / Palm Beach Counties

• Not be currently diagnosed with ESRD (kidney dialysis).

Physicians Health Choice is a Medicare Advantage Organization with Medicare contracts. Members must use contracting network pharmacies and providers.
