Chronic Disease Programs

If you have Diabetes, Ischemic Heart Disease (IHD), Chronic Obstructive Pulmonary Disease (COPD)/ Asthma or Congestive Heart Failure (CHF), you are eligible to enroll in the HealthRight Chronic Disease Programs. Our programs provide educational guidance and care coordination to help manage your chronic condition(s). Our goal is to help patients live a healthier and more fulfilling life.

• We help coordinate your care with your doctor.
• We give you information about how to get healthy and stay healthy.
• We help reduce your medical complications.
• We help reduce unnecessary visits to the hospital or emergency room.
• We help increase your quality of life and well-being.

As a member of the HealthRight Chronic Care Programs, you may qualify for:

Medication Voucher Program (MVP):*

The Medication Voucher Program provides members with medication assistance for some prescription drugs. Benefits include:

• Waived co-payments for certain diagnosis-related drugs purchased from participating pharmacies.
• Based on the Physicians Health Choice health plan, an enrolled member will receive coverage for the entire cost of these specified medications during the medication coverage gap.

Depending on the health condition, active participation in the programs, and service area, members may also qualify for some or all of the following services.

• FREE Education with a Registered Nurse. Includes diet tips, exercise recommendations, and goal setting.
• NO CO-PAY for Diabetic Shoes for members with Diabetes. Includes 1 pair of shoes and 2 insoles/inserts per year.
• FREE Digital Scale for Congestive Heart Failure (CHF) patients.
• FREE Transportation for Chronic Disease Program through Comfort Care. This service is in addition to any existing transportation benefit covered by your Health Plan.**
• FREE Glucose Meter for members with Diabetes. Meter includes a starter kit with 10 test strips.

Physicians Health Choice is a Medicare Advantage Organization with a Medicare Contract. Individuals are eligible to enroll in the plan if the individual is entitled to Medicare benefits under Part A or enrolled in Part B and reside in the service area of the plan. *Drugs are not “free.” Medication costs are counted towards a member’s yearly pharmacy/Medicare benefit. The Medication Voucher Program Drug List is subject to change without notice.
