Prescription Assistance

If you would like to apply for the Low-Income Subsidy (extra help for prescription costs) you can do so by contacting one of the following:

• Physicians Health Choice Social Services Department toll-free at
  1-800-295-3315. We are available to assist you Monday - Friday, 8:00 am to 5:00 pm. (CST).

   

• Social Security Administration at 1-800-772-1213 to apply by phone or to get a paper application. TTY users should call 1-800-325-0778.

   

  Visit www.socialsecurity.gov to apply online

   

• Your State Medical Assistance (Medicaid) office. Call 1-800-MEDICARE, and say "Medicaid" to get the telephone number, or visit www.medicare.gov.

If you do not have prescription drug coverage, you may be eligible to get the medicine you need for free or nearly free. Visit The Partnership for Prescription Assistance at www.pparx.org for more information.
