As a member of a Medicare Advantage plan like Physicians Health Choice, you should not have to file any claim paperwork. However, if you have an emergency or receive care out of the service area, you may receive a billing statement or invoice. If so, members needing to file a claim can mail all copies of records, receipts and any other pertinent information to:

Please note this address is also listed on your Member ID card. All requests for reimbursement will need to be reviewed and approved by your Primary Care Physician. In some cases, reimbursement will be paid according to Medicare allowable, minus any deductibles or co-payments. Please allow 30 days for complete processing.
