To be eligible for Physicians Health Choice Extra HMO Plan, you must:

• Be enrolled in Medicare Part A and Part B.
• Continue to pay your monthly Part B premium.
• Reside in one of our Covered Service areas:

   

  Little Rock Area
  Conway / Faulkner / Grant / Jefferson / Lonoke / Prairie / Pulaski / Saline / White

• Not currently diagnosed with ESRD (kidney dialysis).

Physicians Health Choice is a Medicare Advantage Organization with Medicare contracts. Members must use contracting network pharmacies and providers.
