Our doctors were actively involved in creating this health plan to reflect a patient's needs. We are committed to bringing you more benefits than Original Medicare. When it comes to health care, you do have choices.
PHC is a Medicare Advantage Organization offering five health plans*:
Physicians Health Choice Standard Health Plan is a Medicare Advantage plan for people with Medicare (Part A and Part B).
Physicians Health Choice Select Health Plan is for Medicare Beneficiaries with Medicare and Medicaid.
Physicians Health Choice Extra Plan is an enhanced medical and prescription drug Plan with a monthly premium.
Physicians Health Choice Wellness Health Plan is a Special Needs / Chronic Care Plan.
Physicians Health Choice Basic Health Plan is a Medical Coverage Only Plan (with no prescription drug Plan).

As a Medicare Advantage Organization, PHC has a contract with the Centers for Medicare and Medicaid Services (CMS), the government agency who runs the Medicare program. It is important to remember that Physicians Health Choice is not a Medicare supplement policy. (See Glossary of Terms section for definition of a Medicare supplement policy, commonly called a "Medigap"). As a member of Physicians Health Choice, you must use our network providers. This helps to ensure you pay only the required co-payment amounts for medical services you receive.
